Qualifying medical cannabis patients must register with the Maryland Medical Cannabis Commission (MMCC) through the state patient registry and possess a valid patient identification card. 1. Valid email address. This email address will need to be verified before you are allowed to complete the application process.

- Driver’s license or Motor Vehicle Administration identification card (both front and back)
- Military ID
- Passport
3. Electronic (scanned) proof of Maryland address. Acceptable forms of proof are:
- Utility bill
- Rental contract
- Property tax bill
- Bank statement
- Official documentation from Registrar documenting current enrollment and on-campus living status
FOR NON-MARYLAND RESIDENTS: At this time, the Maryland Commission has placed a temporary hold on accepting applications from out-of-state patients. Please continue to check the MMCC website for any updates or changes to the current status.
4. Recent photo. Acceptable photos must be clear, in color and taken within six months of application submission (similar to passport photos).
PLEASE NOTE: Minor patients (under the age of 18) are required to have at least one caregiver at all times. Only parents and legal guardians of minor patients are eligible to serve as their caregivers. More information can be found on Maryland's official Medical Cannabis website: www.mmcc.maryland.gov.

Once you have been informed via email that your patient registration application is approved, you will need to visit a registered physician (referred to as a “Provider”) to obtain valid written certification to receive cannabis as treatment for your condition.
Approved symptoms for treatment are:
- Severe loss of appetite
- Wasting syndrome
- Severe or chronic pain
- Severe nausea
- Cachexia
- Anorexia
- Seizures or severe or persistent muscle Spasms
- Glaucoma
- Post Traumatic Stress Disorder (PTSD)
As of January 2019 newly registered and re-registering patients must opt to receive an MMCC Patient ID card for a charge of $50. Effective April 1, 2019 Dispensaries will only be able to dispense medical cannabis to patients with a valid MMCC Patient ID card.
